Somatostatin
Indications/Uses
Treatment of intestinal & pancreatic fistulae; excessive secretion from endocrine tumors of the GIT; acute, severe GI hemorrhage; endoscopic retrograde cholangiopancreatography.
Dosage/Direction for Use
Adult 3.5 mcg/kg/hr as continuous IV infusion of 250 mcg/hr. GI hemorrhage 3.5 mcg/kg as bolus inj given after the start of continuous infusion & another bolus 1 min before endoscopy. Fistulae or excessive secretion from endocrine tumors Healing is expected 7-14 days. ½ of the dose infused for the next 48 hr after healing.
Contraindications
Hypersensitivity to somatostatin analogues.
Special Precautions
Severe renal failure. Hypoglycemia. Monitor vital signs & blood sugar. Compromised CV status or history of cardiac arrhythmia. Regular monitoring of renal function & plasma electrolytes is recommended. Avoid abrupt interruption to avoid rebound effect. Pregnancy & lactation. Childn. Elderly.
Adverse Reactions
AV block, bradycardia, arrhythmia, ventricular extrasystole; abdominal pain, diarrhea, nausea, vomiting; hyperglycemia, hypoglycemia; HTN, hypotension, hot flushes.
Drug Interactions
Drugs influencing blood glucose regulation, plasma renin level & arterial BP; any form of sugar including glucose & fructose soln, parenteral nutrition.
